Scheduling Guidelines:

1. When Scheduling Information is received at Raytel Imaging Network, a Scheduling Coordinator will contact the patient and schedule the appropriate study within 72 hours (3 working days) of patient contact.
2. Client will be advised of time and place of study. This will be done via Phone, Fax or Email - client must designate manner in which they want confirmation.
3. Client will receive Radiologists' Report via FAX within 48 hours (2 working days) of study.

Please be prepared to provide the following patient information to RIN’s Scheduling Coordinator:

• Patient Name
• Social Security Number
• Gender
• Date of Birth
• Address
• Home Phone Number
• Work Phone Number
• Referring Physician Name
• Referring Physician Phone Number
• Referring Physician Fax Number
• Test Needed: MRI, CT, Mammogram, X-Ray, Ultrasound, etc.
• Body part(s) to be examined
• HX
• Date of Injury
• Employer
• Insurance
• Claim
