Join the Network

As a state-certified Preferred Provider Organization, Raytel Imaging Network (RIN) has positioned itself as a front-runner among PPOs due to the level of excellence and quality service we demand of our network of diagnostic imaging facilities. To meet RIN's high standards, all facilities requesting to join the Network must be licensed in accordance with all applicable state laws, and all participating physicians must be Board Certified.

To become a Network provider, the following is required:

• Preferred Provider Agreement (PPA) executed by the qualified facility. This can be obtained by calling Provider Relations at 1.800.453.0574, ext. 112.
• Facility Application (a separate facility Application is required for each facility location), This is to be completed in its entirety, signed, dated and returned to RIN with all current facility credentialing documents noted on the New Facility Credentialing checklist.
• Physician Application for each physician at the facility. This is to be completed in its entirety, signed, dated and returned to RIN with all current credentialing documents noted on the Physician Credentialing Checklist.

RIN's Credentialing Committee will consider only applications completed in their entirety and accompanied by all current credentialing documentations for review.
